Member Participation Form

In order to become eligible to receive the pricing and benefits of the Group Purchasing Program, a member or eligible entity is required to complete the member participation form. By submitting this form, your eligibility will be reviewed and once confirmed by Fastenal, you will receive an instructional email containing the relevant features and benefits of the Member Participation Agreement to be registered and acknowledged by you as a member of the Group Purchasing Program.

Administration of this request will take 3 -5 business days.

Submission of this form indicates that this person is authorized to enroll their organization as an eligible member participant in the Group Purchasing Program.
